Infant Airway Health: Building the Foundation for Breathing & Development

    by Dawn Strohschein, DDS

At birth, one of the most important steps in a newborn evaluation is assessing airway health. This early check helps identify any restrictions or abnormalities that could affect feeding, breathing, and growth. Conditions such as tongue-tie or lip-tie may seem minor at first glance, but they can have lasting effects on a baby’s well-being. Tongue-tie, also called ankyloglossia, occurs when the tissue connecting the tongue to the floor of the mouth is too short or tight. This can limit tongue mobility, making it difficult for the baby to move their

These structural restrictions can cause problems from the very first feeding. A newborn must coordinate sucking, swallowing, and breathing in a precise rhythm. If the tongue cannot move freely, the baby may not be able to create a strong seal during breastfeeding. This can lead to difficulty transferring milk, longer feeding times, excessive air intake, or fatigue

Infant Airway Health: Building the Foundation for Breathing & Development

At birth, one of the most important steps in a newborn evaluation is assessing airway health. This early check helps identify any restrictions or abnormalities that could affect feeding, breathing, and growth. Conditions such as tongue-tie or lip-tie may seem minor at first glance, but they can have lasting effects on a baby’s well-being. Tongue-tie, also called ankyloglossia, occurs when the tissue connecting the tongue to the floor of the mouth is too short or tight. This can limit tongue mobility, making it difficult for the baby to move their tongue forward, upward, or side to side. Lip-tie is a similar restriction where the tissue between the upper lip and the gum is too thick or tight, preventing the lip from flaring naturally.
These structural restrictions can cause problems from the very first feeding. A newborn must coordinate sucking, swallowing, and breathing in a precise rhythm. If the tongue cannot move freely, the baby may not be able to create a strong seal during breastfeeding. This can lead to difficulty transferring milk, longer feeding times, excessive air intake, or fatigue during feeding. Over time, this may contribute to slow weight gain, increased fussiness, or colic-like symptoms. For the mother, it can also result in nipple pain, blocked ducts, or mastitis due to ineffective milk removal.
When these feeding challenges occur, it is important to look beyond the surface. A thorough airway and oral function assessment can reveal whether tongue-tie or lip-tie is contributing to the difficulty. Once identified, the next step is building a care team. This may include a lactation consultant to guide feeding strategies, an occupational or speech therapist to support oral motor skills, and a bodywork provider such as a pediatric chiropractor or craniosacral therapist to address muscle tension patterns. In some cases, a referral to a pediatric dentist or ENT specialist for a tongue-tie release is appropriate, but this procedure is most effective when paired with functional therapy before and after the release.
Airway health in infancy is not just about the present moment. The way a baby breathes and uses their oral muscles shapes how their face, jaw, and airway develop over time. If the tongue rests low in the mouth due to restricted mobility, the palate can become narrow and high-arched. This reduces the space in the nasal cavity and can limit airflow. Later in childhood, this can increase the risk of mouth breathing, snoring, or pediatric sleep apnea. These sleep-disordered breathing patterns can, in turn, affect concentration, behavior, and even growth hormone production, which is essential for healthy development.
Speech development is another area that can be influenced by oral restrictions. A child with limited tongue movement may have difficulty forming certain sounds, which can lead to delays or articulation issues. Early intervention not only prevents these difficulties from becoming more complex but also helps children develop strong oral habits that support speech clarity and confident communication.
Identifying airway restrictions early allows for timely interventions that support healthy feeding, breathing, and long-term development.
This approach is most effective when it is collaborative. Each professional on the care team brings a different skill set, and together they can address both the structural and functional aspects of airway health. Pediatricians can monitor overall growth and development, lactation consultants can fine-tune feeding techniques, therapists can strengthen oral coordination, and dentists or surgeons can address physical restrictions when necessary. In some cases, follow-up visits over several months are needed to ensure the child’s airway is functioning optimally as they grow and develop. The key is to provide comprehensive care that looks beyond symptom management to the root causes of dysfunction.
When airway issues are identified and managed early, the benefits can last a lifetime. Healthy breathing supports restful sleep, which is vital for brain development, learning, and emotional regulation. Proper tongue posture and swallowing patterns promote balanced facial growth and a stable bite, reducing the risk of orthodontic problems later. Children who breathe well are less likely to develop chronic mouth breathing habits, which can contribute to dental decay, gum inflammation, and even changes in head and neck posture.
Airway health in infancy may seem like a small detail in the broader picture of child development, but it plays a central role in forming the foundation for physical, cognitive, and emotional well-being. By paying attention to these details at the very start of life, parents and healthcare providers can help set the stage for healthier growth and easier breathing for years to come.
